Poison prevention

Record each medicine dose

Assist only from the medicine label and employer-provided schedule, then record the dose immediately after it is taken.

Critical safety rule

Last reviewed: 2026-09-12

A helper checks a medicine label and records the dose after an older person takes it

Why it matters

One clear, timely record reduces duplicate doses, missed doses, and handover misunderstandings.

Wrong action

Relying on memory, signing in advance, or letting carers keep separate unofficial notes.

Correct action

Check the person, medicine, dose, and time; record only after use and report any mismatch.

What to do

  1. Match the name, medicine, dose, and time to the label and schedule.
  2. Sign the designated record only after the dose is taken.
  3. For refusal, omission, or mismatch, do not make up the dose; tell the employer.

If something happens

If a duplicate, wrong, or excessive dose is suspected, do not induce vomiting or improvise treatment. Keep the packaging and record, tell the employer, seek professional help, and call 999 if serious.
